About Sam Hider Jay Community Clinic

The Jay clinic moved to its present site in the former community hospital facility in 1989. It has a 28,345 sq. ft. floor space. After tribal take over of the existing facility a series of upgrades have resulted in a state of the art outpatient clinic. During FY 2009 there were 45,267 outpatient visits.
Scope of Service:

The clinic provides Medical, Dental, Lab, Radiology, Public Health,
WIC, Nutrition, Contract Care, Pharmacy with mail order, Behavioral Health, Optometry, Community Health Service and Mammography.
Hours of Operation:

Monday through Friday from 7:00 a.m. until 5:30 p.m.
