SLIDING-FEE DISCOUNT PROGRAM
Uninsured patients may be eligible to receive reduced fees through the Sliding-Fee Discount Program. Discounts range from 25 to 75 percent, depending on the financial situation of the patient. Patients applying for reduced fees must provide written documentation of their source of income and proof of residence at the time of service.

Patients must provide at least one of the following documents as proof of income:

Check Stub dated six months or less
Food Stamp Letter
Social Security Letter
Medicaid Card
Medicare Card
Unemployment Letter
Current W-2 Form
Bank Statements
VA
Child Support

Patients must provide at least one of the following as proof of residency:

Mail addressed to patient
Utility bills addressed to patient

Once approved for the sliding-fee scale, patients will be given a patient identification card which will allow them to receive discounts on pharmacy, lab and x-ray services.
